Sit Bone Pain When Sitting Down: Why It Might Not Be Sciatica

Last week, a patient came in and said something I hear often: "My back scan looks completely fine — but I still can't sit through dinner."

When we checked the exact spot, the problem wasn't the lower back at all. It was the sit bone.

This is a pattern I see regularly. Active adults, long-distance commuters, cyclists, and desk workers often spend months assuming their deep buttock pain is a herniated disc or sciatica. But when lower back treatment keeps falling short, the real issue is often right beneath where you sit — an irritated bursa at the ischial tuberosity, more commonly called the sit bone.

Understanding why this happens — and why rest alone rarely fixes it — is usually where recovery starts.


Why the Sit Bone Hurts

Your ischial tuberosity is designed to bear your body weight while seated. To cushion the bone against hard surfaces, a small fluid-filled sac called a bursa sits between the bone and the surrounding soft tissue.

When you're standing, the gluteus maximus covers and protects the sit bone. But the moment you sit down, that muscle slides upward — leaving only thin fibrous tissue and the bursa between your sit bone and the chair. Prolonged sitting, long drives, hard seats, or repetitive hip movement from cycling or rowing can push that bursa past its limit. Over time, it thickens, inflames, and becomes acutely tender to direct pressure.

If sitting on a hard chair, driving for more than 30 minutes, or crossing your legs reliably triggers a deep ache in one buttock — the pressure is landing directly on an irritated sit bone.


Buttock Pain, But Not Sciatica: How We Tell the Difference

Because the ischial bursa sits close to both the sciatic nerve and the hamstring origin, an irritated bursa can easily mimic nerve pain or a pulled muscle. This is why many patients move between clinics without a clear answer.

A careful clinical assessment usually clarifies things:


Ischial Bursitis

Sciatica / Lumbar Disc

Primary trigger

Sitting on hard surfaces, direct pressure on sit bone

Bending forward, coughing, specific lower back movements

Tenderness

Sharp, pinpoint tenderness directly at the sit bone

Lower back or deep gluteal nerve path

Pain spread

Localized to sit bone; may radiate into upper hamstring

Shooting pain past the knee into the calf or foot

Imaging

Spinal scans frequently normal

Often shows disc bulge or nerve root compression


What Treatment Focuses On

In many straightforward cases, the first step isn't imaging. It's a careful history, precise palpation, and understanding how the hip, hamstring, pelvis, and lower back are working together.

When I'm assessing sit bone pain, I'm looking at tenderness directly over the ischial tuberosity, pain reproduction with hip flexion, and whether the proximal hamstring tendon is also involved.


Acupuncture in this context isn't about "curing" bursitis in one session — and it certainly isn't about just numbing the pain.  

When a bursa is irritated, the surrounding gluteal muscles, deep hip rotators, and proximal hamstrings lock up in a protective spasm. This creates a vicious cycle: the tight muscle tissue continuously pulls and presses against the sit bone, keeping the bursa compressed every time you move or sit.

By precisely targeting these hypertonic muscle groups and motor points around the pelvis, acupuncture helps reset this mechanical tension. It reduces local pain sensitivity, improves microcirculation to the surrounding soft tissue, and takes the constant "squeeze" off the sit bone. Once that deep mechanical pressure is relieved, the tissue finally gets the breathing room it needs to settle down — creating a comfortable window where rehabilitation and posture changes can actually take hold.  


For cases where the surrounding tissue has developed long-term fibrotic changes or dense adhesions, Acu-Blade Therapy may be worth discussing as a more targeted option.  


Practical Steps to Protect Your Sit Bone

While treatment is underway, how you sit matters.

A cutout or U-shaped cushion takes direct pressure off the sit bone. Avoiding leg crossing keeps weight distributed evenly. Standing for a few minutes every 30 to 45 minutes helps restore circulation and unload the area. And counterintuitively, aggressive hamstring stretching often makes things worse — it pulls tissue tightly over an already irritated bursa.


When to See a Doctor First

Most sit bone pain is mechanical, but some symptoms need medical evaluation before starting any manual or acupuncture treatment:

  • Fever, chills, or unexplained weight loss

  • Recent significant trauma or a fall onto the pelvis

  • History of cancer

  • Progressive leg weakness or numbness below the knee

  • Changes in bowel or bladder function

  • Severe night pain that doesn't shift with position


Sitting Shouldn't Control Your Day

Sit bone pain can make work, driving, and sitting through a meal genuinely miserable — but it's manageable once correctly identified. If you've been told it's sciatica and treatment hasn't helped, it may be worth looking more carefully at what's actually happening at the sit bone.
